The CAR-OLT score enables healthcare providers, caregivers and patients to estimate 1-year risk for cardiovascular complications after liver transplantation, defined as cardiovascular death or hospitalization due to myocardial infarction, revascularization, heart failure, atrial fibrillation, cardiac arrest, pulmonary embolism or stroke. Risk estimates from the CAR-OLT score are derived from a large cardiovascular risk in liver transplant cohort study published in 2017 in Hepatology ​​(VanWagner et. al. A Point-based Prediction Model for Cardiovascular Risk in Orthotopic Liver Transplantation:The CAR-OLT Score). The CAR-OLT score is intended for use in those age 18-75 years with liver disease who are undergoing evaluation for liver transplantation.

The information required to estimate 1-year post-liver transplant cardiovascular complication risk includes liver transplant candidate age, sex, race, employment status, highest education achieved, hepatocellular carcinoma status, diabetes status, heart failure status, atrial fibrillation status, pulmonary or systemic hypertension status and ventilator status.
